RESUMO

Objective To observe the influence of the clinical pathway management intervention on medical service behaviors.Methods The clinical cases data in a grade A class 3 hospital of Zhangjiakou City,Hebei Province during 2011-2013 were retrospectively analyzed.The influence of implementing the clinical pathway management on the diagnosis results,medical records writing quality,clinical medication,operation,hospitalization time,medical costs,assisted examination items selection were observed and analyzed.Results The admission and discharge diagnosis coincidence rate and preoperative and postoperative diagnostic coincidence rate of 3 diseases entities in the two groups were 100%,the difference had no statistical significance (P>0.05);the medical record grade-A rate in the implementing clinical pathway group was significantly higher than that in the non-implementing clinical pathway group,the medical record grade-B rate was significantly lower than that in the non-implementing clinical pathway group,the difference was statistically significant (P0.05);the average hospitalization time,total costs,drug costs,laboratory fee and examination fee in the implementing clinical pathway group were lower than those in the non-implementing clinical pathway group,the difference was statistically significant (P<0.05);the operation fee,healthy material fee and nursing fee had no statistically significant difference was statistically significant(P<0.05).Conclusion Implementing the clinical pathway can standardize the doctor′s diagnosis and treatment behaviors and makes the medical work to develop to be more favorable for patients.

RESUMO

Objective To analyze the internal and external environment of nursing scientific research management by using SWOT analysis method,so as to promote the improvement of nursing scientific research work in our hospital.Methods Based on the analysis of the strength,weakness,opportunities and threats of nursing staff in scientific research activities in our hospital,this paper draws some conclusions and puts forward corresponding measures according to the analysis results.Results With the continuous development of nursing discipline in our hospital,the construction of the platform was improved,and the nursing scientific research work was improved.However,funding,personnel,consciousness and so on,have become the bottleneck restricting the development of nursing research in our hospital.Combined with the opportunities and challenges,a number of improvement measures are put forward.Conclusions We should strengthen the management of nursing scientific research,improve the relevant systems and measures to improve the internal and external environment,and promote the level and quality of nursing research activities in our hospital.

RESUMO

Objective:To prepare dual anesthetic remifentanil-propofol microemulsion ( Rem-Pro MM) and evaluate the anesthetic effects in adult dogs by continuous infusion. Methods:The Rem-Pro MM was prepared by the high pressure homogenization technolo-gy. The Rem-Pro MM was characterized with the particle size distribution, zeta potential and morphology as the indices. The anesthetic effects in adult dogs were evaluated by continuous infusion of Rem-Pro MM. Results:The particle size distribution, polydispersion in-dex and zeta potential of Rem-Pro MM was (217. 6 ± 48. 2) nm, (0. 212 ± 0. 032) and ( -31. 2 ± 3. 8) mV, respectively. The Rem-Pro MM was found to be small and homogeneous under a transmission electron microscope. The pharmacodynamic studies showed that the Rem-Pro MM in adult dogs with partial spleen surgery had promising anesthetic effects during the anesthesia induction and mainte-nance procedure. Conclusion: The dual anesthetic remifentanil-propofol microemulsion has good effects of sedation and analgesia, which is expected to be used in clinics.

RESUMO

A total of 100 HIN1 flu real-time-PCR positive throat swabs collected from fever patients in Zhejiang,Hubei and Guangdong between June and November 2009,were provided by local CDC laboratories.After MDCK cell culture,57 Influenza A Pandemic (H1N1) viruses were isolated and submitted for whole genome sequencing.A total of 39 HA sequences,52 NA sequences,36 PB2 sequences,31 PB1 sequences,40 PA sequences,48 NP sequences,51 MP sequences and 36 NS sequences were obtained,including 20 whole genome sequences.Sequence comparison revealed they shared a high degree of homology (96%~99%) with known epidemic strains (A/Califomia/04/2009(H1N1).Phylogenetic analysis showed that although the sequences were highly conserved,they clustered into a small number of groups with only a few distinct strains.Site analysis revealed three substitutions at loop 220 (221-228) of the HA receptor binding site in the 39 HA sequences:A/Hubei/86/2009 PKVRDQEG→PKVRDQEA,A/Zhejiang/08/2009 PKVRDQEG→PKVRDQER,A/Hubei/75/2009PKVRDQEG→PKVRDQGG,the A/Hubei/75/2009 was isolated from an acute case,while the other two were from patients with mild symptoms.Other key sites such as 119,274,292 and 294 amino acids of NA protein,627 of PB2 protein were conserved.Meanwhile,all the M2 protein sequences possessed the Ser32Asn mutation,suggesting that these viruses were resistant to adamantanes.Comparison of these sequences with other H1N1 viruses collected from the NCBI database provides insight into H1N1 transmission and circulation patterns.

RESUMO

OBJECTIVE To improve the managerial level of hospital infection by the hospital infection control system through network information.METHODS The control system was installed software in the doctor workstation to realize the real-time monitoring and early warning on hospital infection.RESULTS The passive situation of previous retrospective investigation was changed and the prospective and dynamic monitoring on hospital infection,was realized to move forward the gateway of hospital infection control,facilitate the timely detection on epidemic trend of hospital infection and the clue of outbreak and substitute the previous operations by hands on data processing,statistical analysis and submitting.CONCLUSIONS Full use of hospital information system can promote the hospital infection management.
